Please notify the surgery if you change your name, address or telephone numbers so we can amend your records. We need this information in writing, it can either be emailed to us or you can fill in a form at surgery reception. Please remember to change contact details for your whole family.
If you are currently having treatment with Torbay Hospital it is best to contact them directly regarding a change of details as we do not have access to their records.
